What do I need to do to become a new patient? 
1. Dr. Haddadi is now accepting new patients from birth through age 18 and will be available for appointments beginning October 1st.  Dr. Allen is currently accepting newborns (up to age 2 months) and their siblings or newly adopted/foster children.   Dr. Allen will will be accepting families with older children from a waiting list on a limited basis.  Please call our office if you to register your child as a new patient.

2. If applicable, request a transfer of records from your child's previous primary care doctor.  (We are happy to fax a medical release form on your behalf if you would like to complete it at our office.)

3.  Complete the forms below and bring them to your first appointment or plan to arrive 15-20 minutes early and complete them here.  

4.   Please familiarize yourself with our vaccine policy below. We do not accept families who refuse vaccines or who wish to follow an alternative vaccine schedule.
